Reservation and Circulation Instructions for Participating Libraries

To Reserve a Senior Activity Kit for a Patron:

Fill out a Senior Activity Kit Request Form and have the patron read and sign the Senior Activity Kit Policies form.

For Senior Activity Kit Request Form: give bottom copy to patron, keep middle copy at requesting library, and send remaining two copies to Carnegie Library of Pittsburgh, Kit Coordinator.

For Senior Activity Kit Policies form: give bottom copy to patron, keep middle copy at requesting library, and send remaining copy to Carnegie Library of Pittsburgh, Kit Coordinator.

To Check out a Senior Activity Kit to a Patron:

Call patron to inform of the arrival of the requested kit, or assume the patron will pick it up the day before his/her scheduled program. Check to make sure library card is valid and is the same card number as given on the Senior Activity Kit Request Form. The Senior Activity Kits are not in the system and are not barcoded. You may create an "On the Fly" record or complete a manual checkout. Patron may keep Activity Kit upto 3 weeks. Keep a copy of Senior Activity Kit Request Form with the kit when it circulates. If not returned on time, please notify CLP Kit Coordinator (412.622.3175) who will call patron.

To Return a Senior Activity Kit to CLP:

Send Activity Kit back by courier in the next delivery --do not hold. All Activity Kits should be sent to:

Make sure a copy of the Senior Activity Kit Request Form is still with the Activity Kit.
